REACH is a supportive transition program for young adults who need extra help moving into adulthood, including autistic young adults and youth with social, emotional, behavioral, developmental, mental health, substance use, or juvenile justice-related needs. The program helps participants build real-life skills, confidence, and connection in a safe and supportive setting. Through weekly meetings, community outings, peer support, and guided activities, REACH helps young adults practice communication, self-advocacy, problem-solving, emotional regulation, healthy relationships, job readiness, and independent living skills. The program also helps youth learn how to use community resources, understand their strengths, set goals, and take steps toward a more stable future. REACH is designed to reduce isolation, support smoother transitions after school-based services end, and give young adults a place where they feel accepted, understood, and encouraged. Families also benefit because they receive support, information, and connection while their young adult works toward independence. The goal of REACH is to help young adults feel prepared, supported, and hopeful as they build their next steps in life. |
